Facebook Fan Page App Designer Wanted

Im wanting to find a coder who can build something for my Facebook fan page.
Are you interested? Do you know someone who might be interested? Or can you recommend a company who does this type of work?

Service Required:
Create a tab on my Facebook page.
Have a Poll
Must force the user to be a fan of the page prior to voting
Must collect some data (name,dob,email,mob) prior to voting

We're currently working on our new job board where we are going to promote interaction between job requirements and home grown talent
(rather than going to oDesk or Freelancer etc).
